Task Form Examples

Each Task Form is made up of Elements. The Element type dictates its characteristics. Notice in the examples below that the Titles are essential in directing the staff actions.

Example 1 has three Elements.
Element Types include an Image Upload,  a Text Area Input, and a Checked/See Notes/NA Grid.

Example 2 has three Elements.
Element Types include Radio Buttons,  Dropdown, and Text Area Input.

Example 3 has one Element.
Element Types include a Checked/See Notes/NA Grid.

Example 4 has three Elements.
Element Types include an Image Verification and a Checked/See Notes/NA Grid.

Example 5 has five Elements.
Element Types include an Image Upload, Items in Grid with 1 Column, Image Upload, and a Checkbox.  

Example 6 has three Elements.
Elements Types include two Short Text Input and one Text Area Input. 

Example 7 has five Elements.
Element Types include Text Area Input, Radio Buttons, Checked/See Notes/NA Grid, Video Upload, and Multiple Image Upload.  

Example 8 has two Elements.
Element Types include 2 Photo Uploads.

Example 9 has four Elements.
Element Types include Checked/See Notes/NA Grid, Text Area Input, 2 Photo Uploads.

Example 10 has four Elements.
Element Types include Photo Upload, Number Grid, Multiple Image Upload, and a Checkbox.
